Food safety culture is defined by how people behave when no one is checking, not simply by whether procedures are documented or audits are passed. What happens when nobody is checking?

High-risk manufacturing environments offer relatively little tolerance for error.
Ready-to-eat foods, dairy products, infant formula and other sensitive products can combine complex processing environments, vulnerable consumers, high production speeds, allergen controls, microbiological hazards and strict temperature or hygiene requirements.
Small deviations may therefore have disproportionately large consequences.
Colin explained that many facilities he encounters are highly compliant. Procedures exist. Training records are complete. Audit scores may be good.
Yet an audit provides only a snapshot.
The greater challenge is understanding what happens during routine production when auditors and senior management are not present.
That includes questions such as:
- Are standard operating procedures followed consistently across every shift?
- Do operators understand the hazards behind their tasks?
- Are people comfortable reporting a mistake or near miss?
- Are workarounds becoming normalized?
- Are corrective actions completed quickly enough for the level of risk?
- Do managers spend time observing what actually happens on the production floor?
These behaviors provide a much richer picture of food safety culture than audit results alone can.
Compliance vs confidence: what is the difference?
A compliance-focused food safety system asks:
“Are we following the requirement?”
A confidence-focused system goes further:
“Do we understand why this control exists? Is it consistently working, and would we recognize when something starts to go wrong?”
Both are necessary.
Compliance provides essential structure through procedures, training, verification, documentation and defined responsibilities. Confidence develops when those systems become embedded in everyday behavior.
A mature food safety culture, therefore, moves progressively from:
Reacting to problems → complying with requirements → proactively identifying and controlling risk.
For example, a compliance-driven organization may investigate a non-conformance after it occurs.
A more proactive organization may also investigate near misses, events that could have resulted in a food safety incident but did not.
That distinction matters because near misses provide an opportunity to learn while the consequence is still hypothetical.
Why should food manufacturers monitor near misses?
Near-miss reporting is well established in occupational health and safety, but Colin highlighted an opportunity for food manufacturers to apply the concept more systematically to food safety.
Consider an operator who notices an incorrect label before the product leaves the line.
No affected product reaches the consumer, so there may be no complaint, recall or formal food safety incident.
But something still happened.
Instead of simply correcting the label and continuing production, a mature food safety culture asks:
- Why did it nearly happen?
- Do you know if the procedure was unclear?
- Was the training inadequate?
- Was the process difficult to follow?
- Was line speed creating pressure?
- Could the same issue happen again?
Near misses can therefore provide valuable early signals of weaknesses within a food safety system.
Rather than measuring only what has already gone wrong, organizations can begin identifying the conditions that could allow something to go wrong in the future.
What are the leading indicators of food safety culture?
Many conventional food safety metrics are lagging indicators.
Examples include:
- Customer complaints
- Audit non-conformances
- Failed microbiological tests
- Product holds
- Withdrawals
- Recalls
These indicators remain important, but they primarily describe events that have already occurred.
Leading indicators attempt to provide earlier visibility into behaviors or conditions associated with the performance of the food safety system.

Employee culture feedback
Do short surveys or conversations indicate that people understand expectations and feel able to raise concerns?
The objective is not to replace conventional food safety KPIs.
It is to complement them with measures that provide a clearer view of what may happen next.
Why psychological safety matters in food safety
An operator can only raise a concern if they believe it is safe to do so.
This makes psychological safety an important component of food safety culture.
When an incident occurs, immediately asking “whose fault was this?” can discourage future reporting.
A more constructive starting point is:
“What happened, why did it happen, and what can we learn from it?”
That does not remove individual accountability.
Instead, it ensures that the organization first examines whether the process, training, equipment, communication or working environment contributed to the event.
During the webinar, Colin described an incident in which a relatively new employee responded to an equipment problem. The employee attempted to minimize the immediate impact but did not fully understand where the site drainage systems led.
The important question was therefore not simply whether the employee made the right decision.
It was also whether the organization had provided sufficient induction, hazard awareness and site-specific knowledge for that employee to make the right decision under pressure.
That distinction is fundamental to a learning culture.
Food safety training should explain the hazard, not just the procedure
Employees can complete training while still failing to understand the hazard their task is intended to control.
This is why food safety training should move beyond annual presentations and procedural memorization.
A more effective approach can include short, frequent and role-specific conversations.
Instead of asking an operator to remember that a particular control is required, ask:
- Why do we perform this check?
- What could happen if it is missed?
- Which hazard are we controlling?
- What would make you stop and escalate the issue?
Colin emphasized that employees working within production environments are effectively the organization's eyes and ears.
They see processes operating hour after hour.
Providing them with sufficient hazard awareness enables them to recognize abnormalities that may never appear on a checklist.
The checklist cannot become the objective
Checklists have an important role in food safety management.
The risk arises when completing the checklist becomes more important than understanding what the checklist is designed to verify.
During the webinar, Colin described inspecting areas where employees had repeatedly overlooked obvious hygiene concerns because those specific observations were not explicitly included on their checklist.
The documented task had been completed.
The underlying risk had not necessarily been controlled.
A strong food safety culture, therefore, encourages employees to look beyond individual boxes and ask:
“Does this environment actually look and behave the way a safe process should?”
The same principle applies to standard operating procedures.
Procedures written without sufficient input from the people performing the work can unintentionally encourage workarounds.
Observing processes on the production floor is therefore essential.
Five dimensions to consider when assessing food safety culture
During the webinar, Colin introduced five areas organizations can consider when evaluating the maturity of their food safety culture.
1. Vision and mission
Food safety should connect clearly with what the wider organization says it wants to achieve.
Employees should understand not only the corporate vision but also how their individual roles contribute to it.
2. People
Responsibilities should be clear, while employees should understand that food safety is not solely the responsibility of the quality department.
People also need confidence that raising a legitimate concern will be supported.
3. Consistency
Food safety expectations should remain consistent between teams, departments and shifts.
The required standard should not depend on whether senior management is present.
4. Adaptability
Processes and systems need sufficient structure to maintain control while allowing organizations to learn and improve.
When a new system or technology is implemented, the question should not simply be whether implementation was completed.
Organizations should also ask:
What behavior are we trying to change?
5. Hazard and risk awareness
Employees need to understand the hazards in their own working environment, not merely memorize formal critical control points.
Greater hazard awareness can help operators identify unexpected changes before they develop into larger problems.
